read through the delivery and payment information outlined
below. Once you have read and accept these details, please
click on the button below to place your order.
You
will find two PDF files on the next page. One is a price
list, with product and pricing information. The second is
the Order Form for you to fill out appropriately. Please
use the price list as a reference while filling out the
Order Form.
SUBMITTING
YOUR ORDER
You
have two options for submitting the Order Form:
1.
PRINT & FAX |
Press
the "print" button at the bottom of the
Order Form, and fax to us on (07) 3245 2813. |
2.
SAVE & EMAIL |
Click
on "File" and "Save As." Save
the Order Form onto your computer. Please open your
email account, and attach the Order Form to a new
email. Email to us at gmdaw@bigpond.net.au. |
PAYMENT OPTIONS

Please
enter your card details on the Order Form. Your goods
will be dispatched immediately after confirmation. |
CHEQUES

MONEY ORDERS |
Print
your completed Order Form, and mail to us with your
Cheque or Money order. Your goods will be sent within
5 working days. Please make cheques or money orders
payable to GMD Activewear. |
DIRECT
CREDIT |
Deposit
directly into our bank account - Bank of Qld, BSB

Your goods will be sent immediately after you fax
or email your bank confirmation to us. |
